Key findings from the 2025 results include:
- The largest rate increase was observed in the Colorectal Cancer Screening measure, with an increase of 2.3 percentage points from 2024 to 2025.
- The Optimal Diabetes Care and Optimal Vascular Care measures also had small, but significant rate increases in 2025 compared to 2024.
- The 567xx region (Thief River Falls area) consistently had lower rates than other regions for all mental health measures for both adults and adolescents related measures, including screening, follow-up, response, and remission.
Results for measures reported by health plans and the cost/utilization measures will be updated later this year.
In April, MNCM released two reports related to quality, disparities, and cost for care delivered in 2024:

Aligning for Impact: From Momentum to Action
A 2026 Leadership Series from the MNCM Board Chair & President
Momentum matters most when it turns into action.
When we last wrote in this series, we described the momentum building across Minnesota's health care community. Through the Executive Leadership Council, leaders from every corner of our ecosystem explored what it would take to build trusted, sustainable, statewide health data infrastructure that serves the public interest.
Today, we are proud to share a significant step forward. On October 12, MN Community Measurement (MNCM) convened a special joint meeting of its Board of Directors and members to consider three resolutions:
- Acquisition of the HIE business line. Authorizing MNCM to acquire Koble-MN's health information exchange (HIE) business line and the MNSHINE brand from The Koble Group.
- Change of legal name. Changing MNCM's legal name to Minnesota Statewide Health Information Network and Exchange (MNSHINE), with related amendments to our Articles of Incorporation and Bylaws.
- Establishment of the HIE Advisory Committee. Creating a standing committee of the Board to guide our HIE work and adopting its charter.
The Board unanimously approved all three resolutions, and our members unanimously adopted the name change and related amendments. Following those approvals, our transaction with The Koble Group has closed. MNCM now owns and operates the HIE business line, along with the MNSHINE brand.

A Fond Farewell to Measurement Minute
This is the final issue of Measurement Minute. Thank you for reading, sharing, and engaging with us over the years. Your partnership has made this newsletter a trusted source of updates on health care quality and measurement in Minnesota.
As you’ll read in this issue, our organization is entering a new chapter. On November 1, MN Community Measurement becomes Minnesota Statewide Health Information Network and Exchange (MNSHINE), bringing together statewide quality measurement and reporting with health information exchange.
Our newsletter is evolving too. Starting in January, you’ll receive The Connection, with the same insights and updates you’ve come to expect plus news from across MNSHINE’s work. There’s nothing you need to do; it will arrive in your inbox as usual.
Thank you for being part of this journey. We look forward to staying connected.
Welcome New Board Member - Dr. Justin Golden!

Justin Golden earned his undergraduate degree from Harvard University and his medical degree from Boston University before completing his family medicine residency at the University of Minnesota’s St. John’s Hospital.
Outside of medicine, Justin and his wife are raising four wonderful kids and spend most of their time chasing them around and cheering them on at their many sports and activities. When the kids are sick of him, Justin enjoys running the lakes, drinking plenty of great local coffee, and pursuing his ongoing passion as a decidedly mediocre fly fisherman.
MNCM Completes SOC 2 Type II Audit and Receives Final Reports
MNCM has reached an important organizational milestone with the completion of an independent SOC 2 Type II audit and receipt of its final SOC 2 Type II and SOC 3 reports. These reports reinforce MNCM’s commitment to protecting sensitive information, managing security risks, and maintaining the trust of its partners, vendors, and stakeholders.
Conducted by Johanson Group, the audit evaluated MNCM’s security controls through more than 300 tests, including reviews of access controls, network security, and organizational policies. The SOC 2 Type II audit assessed whether those controls operated effectively over the three-month period from December 1, 2025, through February 28, 2026.
Johanson Group concluded that MNCM’s assertion about the effectiveness of its controls during that period was fairly stated in all material respects, based on the applicable trust services criteria. This independent assessment provides added assurance that MNCM has effective controls in place to support its service commitments and system requirements.
The SOC 2 Type II report provides detailed information about MNCM’s controls and the auditor’s testing and results. The SOC 3 report is intended for public distribution and provides a high-level overview without disclosing sensitive details about MNCM’s systems and security practices.
Once officially released, MNCM’s SOC 2 Type II report may be requested through the MNCM Security Center.
